Collaborative Care and Support Planning Champions wanted

The Royal College of General Practitioners (RCGP) are seeking to recruit champions to be part of a network working with the RCGP’s high-profile Collaborative Care and Support Planning (CC&SP) programme, and NHS England’s Long Term conditions team, to support general practice teams and CCGs to implement and understand care and support planning and person centred care.

CC&SP is a process for improving the conversations between people with LTCs and their health care team. Patients, and carers, are empowered to become equal partners with health care professionals to co-produce a care plan shaped by their personal goals. The result is a single, person-centred care plan that encompasses all their mental, social and physical needs; supports self-management where appropriate and leads to better continuity and integration of care. To support the RCGP/NHSE in achieving our goals to promote person centred care and care and support planning into general practice, we are recruiting a network of champions. We are actively recruiting champions from a range of professionals and welcome social workers, nurses, doctors and other clinical specialists. The network will be fundamental to making this a reality on the ground.
