This three-year, Scotland-wide project involved three phases. Phase one focused on a review of international literature on transition. Phase two involved qualitative interviews with 10 parents of young people with complex learning disabilities and 46 nurses and other practitioners from all 14 NHS Boards in Scotland. Their experiences of transition from child to adult health services were used to develop best practice strategies for transition. Phase three involved developing an online educational resource on transition and piloting it with 12 registered nurses from two NHS Boards.
